In this timely expose of "tension between . . . medicine as a profession and as a business," professors Jesilow, Pontell, and Geis (the latter emeritus) of the University of California's School of Social Ecology contend that since the 1965 establishment of a reimbursement of Medicaid fee-for-service policy, a "marginal" group of physicians, in addition to fee-splitting, has been multiplying superfluous visits, needless tests and procedures, and more. (Annual cost of unnecessary surgery in the U.S. is estimated here at $3.92 billion.) Citing case files and extensive interviews with both private medical personnel and government officials involved in enforcing Medicaid and Medicare fees, the authors discuss AMA attitudes, insurance, fraud detection, penalties (suspension, fines) and how guilty doctors tend to rationalize their misconduct. And, anticipating a U.S. national health serivce, the authors appraise those of other countries.

Since the federal government began determining fees and regulating treatments through Medicaid in the mid-Sixties, some healthcare providers have blatantly taken advantage of the system and fleeced the American public. Jesilow and other colleagues in the School of Social Ecology at the University of California, Irvine, report on doctors, pharmacists, and insurance companies who have been convicted or who have confessed to Medicare crimes. The authors expose the greedy and self-serving behavior of these outlaws, estimated to cost between 10 and 25 percent of the total program cost for state and federal governments. The authors conclude that changes can come about only through a basic overhaul in the medical care delivery system--an issue that is on the front page of newspapers daily. Timely, well written, and carefully documented, this book is sure to keep the reader's interest.

A hard-hitting, well-documented expos‚ of how physicians milk the Medicaid system. The cost of Medicaid fraud is unknown, but every year about two hundred physicians are suspended from the program because of fraudulent or abusive practices. The authors (all professors at the School of Social Ecology at the University of California, Irvine) look at these violations and examine the system that has provided the opportunity for them as well as the backgrounds of doctors who commit them. (Some of the violations are outrageous: bills for therapy while the therapist, a psychiatrist, was having sex with a patient; bills for abortions on nonpregnant women.) Medicaid's fee-for-service system makes fraud easy: By simply checking a box on a form, a doctor (or a billing clerk) can charge for services not performed. Meanwhile, ripping off the government apparently doesn't create guilt feelings: Interviews with 42 physicians reveal that these M.D.s see themselves as the innocent victims of bad laws and bureaucratic interference. And the medical fraternity has taken a relatively benign attitude toward Medicaid fraud, with state regulatory boards rarely revoking licenses of apprehended physicians. The authors offer no quick solution to the problem, noting that tension between government regulation and professional norms of behavior is growing. Finally, they look at the health-care systems of Great Britain, Canada, and Australia, to consider how they cope with fraud, concluding that the problem can be solved only through structural changes in the delivery of health-care services. Exactly what changes are necessary remains unclear. Combined Insurance goes further than McDonald's in attempting to standardize the workers' very selves, instilling in them adroit maneuvers to overcome customer resistance.

The routinization of service work has both poignant and preposterous consequences. It tends to undermine shared understandings about individuality and social obligations, sharpening the tension between the belief in personal autonomy and the domination of a powerful corporate culture.

Richly anecdotal and accessibly written, Leidner's book charts new territory in the sociology of work. With service sector work becoming increasingly important in American business, her timely study is particularly welcome.

Leidner (sociology, Univ. of Pennsylvania) uses participant observation to explore aspects of service-industry efforts to insure sameness of effort and routinization of work. The author chooses for examples the ubiquitous McDonald's and the Combined Insurance Company, whose founder, W. Clement Stone, formulated the Positive Mental Attitude (PMA). Both companies achieve service provider-service recipient relationships that are routinized yet acceptable both to the customer and employee. The much-touted "worker rebellions" are largely nonexistent; many if not most employees prefer a well-choreographed approach to the point of sale. Leidner's book includes much of interest to students of business and human behavior, but her turgid prose does not lend itself to easy reading. For academic libraries.

Engaging book, debateable premise

When all is said and done, American consumers are more alike than they are different. This truth of human psychology provides the bedrock of our mass production economy; it is possible to create core products that millions of people will buy, and it is possible to design methods of presontation and marketing that millions of people will respond to. Consistancy beats creativity. That said, individuals, even those who aren't considered wildly creative or in any way eccentric still need to shave off 5 - 10% of their personalities to find their fit in cultures devoted to providing a consistant product or approach to business.

Dr. Leidner's study of how this morphing of individual to group collective focuses on two job classes at diverse ends of the American economic experience. On the one side is McDonalds, whose counter and burger flipping positions, even if in many instances filled by students on their way to something far different, have also become a shorthand for employment of last resort among those with few skills, options, and long term prospects. On the other end of the study is Combined Insurance Company of America (CICA), a subsidiary of Aon Corp. CICA is a sales driven company, and sales - dispite the field's high washout rate - remains for those who do it well the highest paying profession in America.

Early on Dr. Leidner suggests that there is irony in the fact that these companies, which rely on structure and standardization to a degree uncommon in their respective fields, were formed by highly dynamic individualists, Ray Krock and W. Clement Stone respectively, who challenged convention and relied on personal instinct in building their empires. It makes for a nice sentence or two, but most people who build empires, be they Krock and Stone, or Henry Ford, Andrew Carnegie, James Hill, Michael Dell, Sam Walton, etc use a different approach to building than will subsiquently be needed to maintain and expand those companies. Even a company as wildly creative as Apple Computer needs to instill a degree of structure within the organization if only so it doesn't fall prey to entrophy.

Still, organizational struture isn't the theme of the book, instead it is what happens to people whose jobs involve working directly with customers, human interactions, that realm of life where individual personality is most apt to express itself, but who are told to conduct those exchanges with the same routine and scripting of a working standing at a machine and mass producing parts on an assembly line. Because McDonalds is a recognizeable American icon most of the attention given this book - including the other review on this site - focuses on the chapters dealing with the fast food giant. I found the chapters about Combined Insurance Company far more interesting. I work in sales and during my second year in the field I joined (and have since left) CICA in Virginia during the time that Leidner was doing her research and observations in the company's midwestern operations.

Leidner focused on the company's Life divison, even as she stated that the Accident Indemnity divison which I sold in, practiced the greatest reliance on standardized presentations and scripting in general. In her assessment the company's philosophy of reliance on a tightly structured sales system came at the expense of individualism and forced sales reps to subliminate elements of their own personalities and as such had to struggle with feelings of inauthicity and a loss of self.

To a degree her assessment makes sense, and as CICA gave her full access to the company's sales school, as well as interivews and field time with new agents and managers, her studies did reflect what her subjects cited as their actual experiences. That said - and dispite CICA giving her more cooperation and acceptance than some at McDonalds gave her - I feel that she didn't do as complete a job of putting CICA in its proper place within the evolution of sales methods over the past 100+ years.

Dr. Leidner seemed at times to subscribe to the myth of the 'natural born salesperson'. The hired gun who shot from the hip, said whatever came to mind, and sold circles around everyone else b/c of inate abilities that couldn't be studied, quantified, or taught. This misconception has long bitten at the heals of anyone who dared argue that sales can be taught. In the 1890's when John Henry Peterson organized the first sales schools for NCR reps, and wrote the first standardized sales manual, which he demanded his reps follow - at the risk of being fired even if they met quota while using other methods there has been a debate over whether selling is a learnable skill or an artform that one either has or hasn't. People in the latter camp react to any approach to standardize the field in the same way a literary writer reacts to romance and mystery novelist who churn out and sell formulaic fiction by the boatload.

It has been my observation over 20 plus yrs that people who enter sales with the latter view often washout and return to other fields when they find that doing it by the seat of their pants doesn't work. CICA's training wasn't so much about denying indiviual initiative, but about giving the individual agent tools that worked (this afterall is the field where Elmer Wheeler, the man remembered for coining the phrase 'sell the sizzle, not the steak' found that a sales rep could increase his sales 500% just by omitting two words in his closing sentence).

Reading Dr. Leidner's account of new hires trying to deal with CICA's methods in 1987 I was reminded of some of my long forgotten impressions from sales school. At the time I watched my classmates, most of whom were new to sales and with little previous knowledge of the history of the field, its inovators, and groundbreakers, or of the principles of human behavior that drove the methods, react defensively and with uncertainty to what they were being taught. My impression at the time was that CICA could have generated greater acceptance of its methods if some time had been given to validating the underlying principles - which that minority of us with prior sales backgrounds knew to be true. Dr. Leidner's interviews suggested that some of the new agents she spent time with struggled with the same thing.

As such, what she terms a stuggle to keep subverting the self, and losing one's individuality to scripted methods, may in fact have been an individual lost in a tool chest or weighed down by a tool belt such that they somehow failed to see themselves for what they really were, craftsmen provided with world class tools, whose success or failure would depend on how they as individuals used those tools.

A Prophecy Fulfilled?

Hungry and tired of the banality of couscous, hara (a tomato based soup) and tajine that I had been eating in Morocco for the past four months, I burst through the doors of one of the McDonald's in the city of Casablanca. I was on a mission. I had dragged several of my friends by train, bus, taxi, and foot for the singular purpose of eating a Big Mac. I was feed up. I needed food that I could trust to be produced in some standardized fashion, that would be clean and well cooked. I also needed to use a bathroom that was clean. This "quest" occurred during a foreign exchange to Morocco during the fall of 2000. But the story begins much earlier perhaps in 1994 when I was 12 or 13. My father had taken me to our local Borders bookstore in the suburbs of Philadelphia to hear a lecture about McDonald's of all things (I was home schooled at the time and going to lectures such as these was a continual part of my education). Robin Leidner's lecture was probably my first exposure to sociology and I believe one of the causes that lead to a radical shift in my adolescent thinking. Never again was the world the same. In a sense, Robin Leidner gave birth to my "sociological imagination." Ever since I have also been fascinated by McDonald's as an indicator of American culture. There is something awe inspiring about the success of McDonald's and its guarantee of a certain quality of food, cheer, and service no matter where in the world I should travel. (The phenomonem of McDonald's seems reminscent of something one might find in a dystopian post-industrial science ficiton story.) That at, one time, 7% of US workers had worked at McDonald's at one time or another, speaks to the influence of McDonald's in the lives of Americans. On the other hand, the routinization that has been key to McDonald's success raises questions of alienation, autonomy and robotization of employees for the sake of profit. Ever present is the conflict between self-identity and existence in a corporate world that requires all things to be standerdized. Dr. Leidner provides insight into corporate culture that many of us will at sometime or another participate in. It would be good judgment to know what we are getting ourselves involved in. I, for one, have learned to appreciate the significance of McDonald's organizational structure, while being cogent of the impact of its corporate values on the individual and society. There is a sense of empowerment in "decoding" society and locating ones position in society and it is my hope that others, outside of academia, will read Leidner's book and become consciouse of their organizational context. There is the sense of a prophecy fulfilled. After first seeing Dr. Leidner and reading of the expansion of McDonald's to Morocco, I have, seven years later eaten at the first McDonald's in Morocco, always with "Fast Food, Fast Talk" in mind. The impetus for this has always been to see for myself if Dr. Leidner was right: could McDonald's really be the same everywhere? It would seem that my life has already come full circle. As the result of my first exposure to sociology from Dr. Leidner, I am currently studying the sociology of work and occupation!

Phenomenal

I wish I had read this great work when a graduate student in my sociology program. This is a very insightful and scholarly work that is directly applicable even today to those in many professions who have been experiencing the negative effects of depersonalization, alienation, deskilling, routinization and other sordid effects of various occupational ghettos (such as libraries) that were never that great to start with but have since rapidly declined in their so-called "professionalism" that at least required some technical skills and knowledge to a little degree. This is a must-read for public librarians especially; those in the stressful food industry; and those studying the illustrious social sciences. Most highly recommended. Hope to see more books for Dr. Leidner. Should be required reading in all library science graduate programs.

Perched among peaks that loom over heat-shimmering plains, hill stations remain among the most curious monuments to the British colonial presence in India. In this engaging and meticulously researched study, Dane Kennedy explores the development and history of the hill stations of the raj. He shows that these cloud-enshrouded havens were sites of both refuge and surveillance for British expatriates: sanctuaries from the harsh climate as well as an alien culture; artificial environments where colonial rulers could nurture, educate, and reproduce themselves; commanding heights from which orders could be issued with an Olympian authority.

Kennedy charts the symbolic and sociopolitical functions of the hill stations over the course of the nineteenth and early twentieth centuries, arguing that these highland communities became much more significant to the British colonial government than mere places for rest and play. Particularly after the revolt of 1857, they became headquarters for colonial political and military authorities. In addition, the hill stations provided employment to countless Indians who worked as porters, merchants, government clerks, domestics, and carpenters.

The isolation of British authorities at the hill stations reflected the paradoxical character of the British raj itself, Kennedy argues. While attempting to control its subjects, it remained aloof from Indian society. Ironically, as more Indians were drawn to these mountain areas for work, and later for vacation, the carefully guarded boundaries between the British and their subjects eroded. Kennedy argues that after the turn of the century, the hill stations were increasingly incorporated into the landscape of Indian social and cultural life.

Was a Nice Introduction to Maya Glyphs 20 Years Ago.

"Maya Glyphs" is part of the British Museum's "Reading the Past" series that introduces readers to ancient scripts. These are all slim volumes that are intended only to explain in general terms how a script developed, how it works, who used it and how, with examples. Even so, the "Maya Glyphs" volume is weaker than the others in the series, because the script is still in the process of being deciphered, which renders some of this volume obsolete, and because Maya glyphs were extremely complex. It is difficult to convey how they work in such a short space. None of this is the author, S.D. Houston's, fault, but readers may be happier with the more recent "Reading the Maya Glyphs" by Michael Coe, which Houston himself recommends.

"Maya Glyphs" is not without value, however. Maya glyphs are the written version of languages that are 4,000 years old now but which did not achieve literacy until perhaps the third century. Their golden age was during the 3rd-9th centuries, after which the written language declined and was entirely forgotten by the 18th century. During its peak, about 5,000 texts were written in Maya glyphs, either carved or painted. Modern man was ignorant of their meaning until a renewal of interest in the glyphs in the 19th century set epigraphers on the road to deciphering them. When this book was published in 1989, 60% of the glyphs had been deciphered, and points of grammar are still contentious. Houston takes us through the progress of the 19th and 20th centuries.

Then he introduces the reader to the structure of the language, which combines logographs and phonograms. Houston attempts to explain how the glyphs are employed: in compounds, as affixes, main signs, and phonetic syllables. There are also ideographs, which are poorly understood. Scribes could write the same word or syllable many different ways. Houston isn't kidding when he calls Maya glyphs "one of the most complex scripts ever devised". I don't think there was much hope for widespread literacy with this script. It's interesting that the Mayans used their written language to record history, not to make financial accounts, which is often the impetus behind written language. It's not that they didn't like numbers. Mayans were preoccupied with calendars, which the author explains in some detail.

Where do these people come from?

As the author of this slim book I should probably respond to the annoying review by Kuhn. First of all, the book's length was established by the British Museum Press -- I had nothing to do with this decision. "Maya Glyphs" forms part of a series, all of which are volumes of equal length. Second, the manuscript was prepared in 1988. Obviously I would not write precisely the same book today. The Press has never allowed me to revise the book, and it certainly needs to be replaced. Mike Coe will soon publish, with Mark Van Stone, an introductory volume that should please and instruct readers. Most other books on Maya glyphs cannot be recommended at this time.

Not much value here

Sadly, I can find very little to recommend this book for. Consisting of barely 80 pages, it starts with a short introduction of the nature of Maya writing and its decipherment, both of which you will find in more comprehensive and understandable form in Michael Coe's "Breaking the Maya Code". It then switches very fast to some rather specialist examples completely unsuited for the beginner. In the end, you get 1 (read:one) sample text, and that's about it. On the positive side, which is as thin as the book itself, Houston mentions some interesting details, like the stylistic variations among glyphs of different regions, which are rarely found in other volumes, but these tidbits of valuable information hardly constitute a reason to purchase this book. One for completists only.

Quite good

This book was quite good but the best part was the behind the scenes view of cinema in the 20's. It is quite well written and focus on the genius of the director. I quite enjoyed it bu it would have profered more on the actual films of DeMille rather than the backround. Also i did not think it had a acurate portryal of some of his helper and really neglected to mention the people who helped Demille become who he was.

Perfect book for my purposes

We just hired a nanny from Ethiopia and her English was minimal. I got this so that she could work on improving her vocabulary in her free time. I also got a monolingual picture dictionary, but she seems to use this one more than that. The book is easy to use as it goes both ways (Amharic-English/English-Amharic). Nice to know that amazon.com is carrying titles like this. Keep it up!

Best Dictionary Out There

I learned some Amharic from Ethiopian friends at high school and college. I was rather fascinated by the deep rich culture. This dictionary is just plain amazing. It includes both English-Amharic and Amharic-English sections, and utilizes the actual Ethiopian alphabet and phonetic guide.

For an English speaker, I would recommend getting copy of the alphabet displayed in grid format, and run though memorization of the alphabet with an indigenous speaker. This way, you'll be able to utilize the dictionary to the fullest potential.

A must read for activists

This book is absolutely essential reading for any activist. The author, who is a successful activist himself, covers everthing you need to know to get your cause "out there" where people (and especially decision-makers) will see the importance of your issue and choose to do something about it.
If you are an activist that wants to be both efficennt and effective, I would highly suggest reading this book

Essential reading for serious social-change activists

This book provides concrete examples of how grassroots social change organizations are either successful or unsuccessful in reaching their goals. Case studies of actual campaigns are presented and particular strategies are analyzed in various fields of activism. Legislative strategy, ballot initiatives, direct action and dealing with the media are all presented. Compelling reading and very rewarding, this book is also the activism Bible of the adult adoptee-rights organization Bastard Nation.

Good strategies, but where's the democracy?

Good advice especially re: dealing with the press, lawyers, and applying strategy. Would have liked to see a lot more about getting social change organizations to really involve their members in running the organization- its still "Tales of the Super Organizer" a model activists need to outgrow and are already outgrowing.
